GBM, IDH-mutant glioma, meningioma. TTFields (Optune) now standard for GBM. IDH inhibitors (vorasidenib) showing benefit in lower-grade gliomas.
CDK4/6 inhibitors (palbociclib, ribociclib, abemaciclib) standard for HR+/HER2−. T-DXd preferred for HER2+ metastatic. Olaparib/talazoparib for BRCA1/2 carriers.
AML: ven+aza standard; FLT3i (gilteritinib) and IDH1/2i per mutation. CML: asciminib (STAMP) superior to TKIs. CLL: ven+obinutuzumab or BTKi (acalabrutinib, zanubrutinib).
